We are excited to partner with the Grant County Health Department and Community Counseling Solutions to offer an on-site, school-based health center. The Prairie City School-Based Community Clinic is open to everyone—students, faculty, and community members alike—reflecting our commitment to accessible healthcare for all.
Staffed by the Grant County Health Department and Community Counseling Solutions, the clinic provides comprehensive services, including:
• Office Assistant
• Nurse Practitioner
• Certified Medical Assistant (CMA)
• Community Health Worker (CHW)
• Qualified Mental Health Associate (QMHA) or Qualified Mental Health Professional (QMHP)

The Community Clinic offers a variety of primary care and behavioral health services, including:

· Vision, hearing and dental screening
Supports & Wellness
· Health Education & counseling
· Fitness & nutrition guidance
Behavioral health services
· Individual and Family Counseling
· Behavioral health screening and treatment
Referrals for services we do not have onsite are provided as needed.

Hours: Open year-round. Mondays - Thursdays 8:30AM-4:30PM

School-Based Health Centers are funded through a combination of third-party insurance, Medicaid, grants, and public funds. Services are billed to insurance providers for those with coverage, in accordance with legal requirements. Our top priority is ensuring student access to care in a convenient, youth-centered environment—regardless of ability to pay. We accept private insurance, Medicare, Medicaid, and the Oregon Health Plan. For those who are self-paying, a sliding fee scale based on income is available.

For urgent care questions outside of clinic hours, please call 855-750-5068. Please note that this nurse advice line is for urgent health concerns only and they cannot refill medications or access personal medical records.
